Did someone mine out the entire thing, stone and all?
You have to leave at least some stone for it to regenerate. If it gets completely mined out then it won’t.
Sadly it’s a limitation of the regenerate abilities of the game.
Same goes with POI’s, if every last block and device gets removed it can no longer regenerate.
