Did you have success by now?
What for most worked is to disconnect all your discord accounts. You probably have logged in to Discord also with your web browser or smartphone?
After that clear the cache, login to patreon, disconnect your discord sync, and connect again.
I can give it manually but the bot will handle it over time as well.
