It looked like maybe you all were still making changes to this issue. I just wanted to chime in again with what I noticed on my end.
I died at 21:25 server time. At that time, my survival bonus did NOT go away. I logged for a while and when I returned I noticed my skill tree was reset (08:30) and survival bonus was gone. I have since played a few hours and am accruing survival bonus as expected. So as of this morning everything appears to be working right =)

Since changes were made it maybe needed some hiccup at the first death.
Hopefully it works from now on correctly.
